I think the answer is B. The CEO is the head of both hierarchies, so there's no need to fill in the MANAGER or SECOND_MANAGER fields. Leaving them blank makes the most sense.

For the CEO, the MANAGER should be blank and the SECOND_MANAGER should be blank. There's no one above the CEO in the hierarchy, so those fields should be left empty.
