Hold on, is the CEO actually the head of both hierarchies? I was under the impression that the CEO was the top dog, so maybe option A is the way to go here.

Option D makes the most sense to me. The CEO has no manager, so the MANAGER field should be blank, and the SECOND_MANAGER field should be 'NO_MANAGER'.

I'm not sure about that. I think having the MANAGER as blank and SECOND_MANAGER as NO_MANAGER could also work, as long as the CEO is at the top of the hierarchy.
