I believe they are only sent to the Fail output.

This is like a game of 'Guess Where the Records Go'. I'm going with option D - to both the Pass and Fail output, just to see the system explode!

Hmm, let me think... If both rules fail, the records must be sent to the Fail output, because that's the only logical conclusion. Unless, of course, it's a trick question. In that case, all bets are off!
