I'm not sure about option B, wouldn't that allow more than two devices per port? I'd go with D as well.

Option B does seem like it could allow more than two devices. D looks like the safer choice.

Option D looks good to me. The requirement states one VoIP phone and one device, so limiting the client count to 1 and using device-mode authentication seems to fit the bill.
