Hmm, this is a tricky one. I'd say it's a combination of B and D. The mirroring and SSL offloading issues seem to be the root cause.

D sounds the most plausible to me. The lack of SSL offloading on the load balancer could be the culprit here.

Ha! Looks like the load balancers failed to balance the load properly. I'd go with A - the clients were sending requests to the failed load balancer.
